Tax forms question-1099 income

I need to file federal for 2003. A small portion of my income was
normal w-2 catagory. Most was 1099 income, working out of state. I
have a ton of receipts. Which forms and instructions do I need to
properly complete my return? Thanks in advance.

Form 1040, Schedule C, Schedule SE, Form 4562

All appropriate forms and schedules relating to each and every state you
worked in during that year.
